I attended Axe-con 2026 and wrote up my takeaways. Three things that stuck with me:

Greg Gibson from Red Hat built a page that passes every automated accessibility scan with zero errors. It's completely unusable with a keyboard. He then tested cloudflare.com live on stage and found issues in minutes. Six keys, fifteen minutes - that's all it takes.

Meanwhile, Microsoft measured how well AI generates accessible HTML. Without accessibility instructions: 10% WCAG pass rate. With detailed custom instructions: 58%. GitHub responded by building accessibility documentation specifically for AI tools to consume.

What the case studies had in common: encode expectations into systems - career levels, design patterns, conformance reports - not documents. And accept that "better is better."
